Daptomycin Injection Daptomycin is  restricted for use in microbiology proven gram-positive skin and skin structure infections resistant to vancomycin and linezolid or that have failed vancomycin/linezolid therapy, or for patients who are allergic to vancomycin and linezolid.
Linezolid Injection Linezolid is restricted for treatment of documented serious vancomycin resistant Enterococcus faecium infections when no other alternative is available.

Doxycycline and nitrofurantoin are recommended for susceptible VRE lower urinary tract infections.

Linezolid is not recommended for treatment of other documented infections unless the patient is allergic/intolerant to all other antibiotics demonstrating activity against the pathogen.

Vancomycin Capsules Non formulary, auto substitute vancomycin oral solution made from vancomycin injection.

Recommendation: MEC approved.

 Procedure:

1.         Reconstitute the 500 mg vial with 10 ml of sterile water, giving a 50 mg/ml concentration.

2.         Withdrawn 5 ml for a 250 mg dose or 2.5 ml for 125 mg dose of vancomycin.

3.         Further dilute the dose to 30 ml with water or cherry syrup before administering to the patient. Note vancomycin injection is essentially tasteless.

4.         Unused portions of the vial may be held for 24 hours at room temperature before discarding. Note vancomycin is stable for greater than 7 days at room temperature.
